Darlene, I purchased Lactation Forms in 1993 at a LLL
of So. Cal. conference (both Chele Marmet & Ellen
Shell were speakers).  I suspect you'll find it for
sale at any conference where one of them makes an
appearance as well as through The Lactation Institute.
 My 1993 copy lists the following address:

Undoubtedly, someone will have more current
information (perhaps phone #, website, etc).  If there
is an updated edition, will someone post all relevant
info?
