I think the deep crack points to an opening for infective mastitis, but
positioning can definetely contribute to the crack in the first place.
Also the fact that this mom "gets better" to only relapse points to not
really getting all better.  Using antibiotics for a few days until better
instead of the whole course is classic cause of relapse.  Also  makes the
