I've a paper on long-term breastfeeding coming out in Breastfeeding Review 
in March. What I found is that being "shocked" and "appalled" by seeing a 
non-infant breastfeed is a necessary part of the process though which women 
end up breastfeeding beyond infancy themselves. It's not a bad thing, it's 
actually a great thing.
I asked our local Child Health unit to put up a poster of a toddler 
breastfeeding in their waiting room. They agreed and whilst some were a bit 
shocked by the poster it's still there and I am sure that it has an impact 
and becomes ho hum over time and repeated exposure.
