I have learned (the hard way) that you can't always see a tongue-tie,
unless you use a gloved finger to lift the tongue up, away from the
floor of the mouth.  It is easier to feel the really posterior and
nonelastic lingual frenula as you run a fingertip under the tongue, then
look more closely after you identify it by feel.  I clipped a 2
month-old recently who had a very insignificant appearing
tongue-tie...about 2 mm deep at the base of the tongue.  The mom had
instantaneous relief of the pain she had endured for 2 months.
Unfortunately you often cannot tell by looking which ones would benefit
from frenotomy...but if mom is in pain, it is likely to help.
