The "choice" to return to work doesn't always rest with the new mom.

Many moms return to work for the insurance.  A lot of dads here are
self-employed, and they have their health insurance thru mom's work.  If mom
doesn't return to her (often low-paying) job, they don't have insurance, and
can't really afford it, either.
