Diane's client has a lot of serious physical stress going on.  Pneumonia is
a difficult illness to bounce back from, and on top of the diabetes, this
mother may just not have the metabolic energy to make milk.  I would expect
that to improve as she recovers.  The way I always manage these moms is to
ask them to pump without getting attached to the vols (which are bound to be
low.)  If they can just keep their breasts "turned on" the milk supply
should slowly rise as their physical condition improves.  The mother will
need lots of household help, reassurance, etc. I can't think of anyone I'd
want more in my corner during such a recovery than you, Diane.
