has this Mum had a swab taken of the cracked areas on the nipple and ?  
a sample of milk sent for mcs? The cracks you mention and the fact her  
pain has been unresponsive to antifungal treatment, make me wonder if  
this Mum may have a staph bacterial infection. Cracks can provide a  
great portal for bacteria into the ducts. Apologies if this has  
already been discounted,
