I received a call this morning from a mother who is feeling desperate about
her baby's reflux.  Here is the situation:

The baby, who weighed 7 lb. 3.6 oz. at birth, is now seven weeks old.  The
mother has exclusively breastfed since her baby's birth.  The baby had
projectile vomiting from the beginning,  often several times a day and even
during feeds.  At three weeks the baby had an upper GI that ruled out
pyloric stenosis.  At that point he was put on ranitidine .3 cc. and
cisapride .3 cc. every twelve hours.  The mother noticed marked improvement
in the baby after he started the meds and he began to put on weight.  At
five weeks he weighed 7 lbs. 10 oz.  Things continued to go well until
Monday when his projectile vomiting returned.  A weight check today
revealed a 2 oz. gain in the last two weeks.

The pediatrician, who is extremely concerned about this baby's poor weight
gain, is recommending the following:

Continue meds, breastfeed and follow up each feed with 1-2 oz. bottle of
ABM thicked with cereal.

His rationale is that the baby will gain weight with the ABM
supplementation and will keep the ABM and breastmilk down better if the
cereal is mixed in.

The mother and baby are leaving town tomorrow for a week's vacation and she
has reservations about following her doctor's advice.
