The problem here is the writer is holding the back of the baby's head.
If mom scoops up the baby's rear end with her forearm just in front of
the elbow and presses his tummy against her chest under her other
breast, baby is usually all lined up for an asymmetrical latch with the
nipple by his nose.  If mom's hand is on baby's neck and shoulders where
it should be, all she has to do is gently push with the heel of her hand
between his shoulder blades when he opens up, and he's on, head slightly
extended for better tongue contact.  If she leans back, gravity helps
hold him in place so she does not fatigue.  If she has to lift baby to
the breast, she's holding him too high.
