If the latch is good, the other issue here could be excessive pressure 
on the nipple due to reduced milk supply. Infants usually exert more 
pressure on the nipple when milk supply is low, in this case from the 
surgery. The extra pressure cuts off blood flow, reducing perfusion. 
This can cause reflex vasospasm after each feeding.
Using a Lact-aid or SNS might help.
If the latch is poor, excessive compression of the nipple is inevitable.
