I was talking with a couple other LCs who had not heard of using soap and water to cleanse a wounded nipple. The recommendation they were familiar with was expressed breastmilk and normal saline soaks, and that soap and water were “drying.” I found some info on Dr Newman’s site and some other sites but not sure if we have evidence from a comparative study or anecdotal evidence. I know we have discussed on lactnet before. And I am talking about a wound, not just a tender nipple in the early days of breastfeeding. I did see one site that mentioned that many LCs recommend soap and water and that it keeps a “biofilm” from forming.
I wanted to provide some more solid evidence so that we could discuss further and possibly other newer treatments they are not aware of like Manuka honey. Does anyone have more solid evidence for soap and water cleansing or do any of  you recommend that when the wound is beyond “routine” sore nipples?
