I'm looking at this problem from a slightly different angle and wondering if
it's the need to be 'doing something' at the breast besides drinking? Maybe
it's this little guys hands that need to be kept busy during the feed?
I had this with my middle one (now 3). Also around 4-5months he would be
stuffing fingers into mouth (also got his first teeth around this time) when
latching, so I started giving him my watch to play with while nursing. This
helped, and soon I was going around with a bag of 'bits' for him to play
with. Not normal toys for this age, but things like little lego dolls or
small dinky cars (the wheels of which kept him busy while drinking). He
actually grew out of this at around a year and it was never a problem
anymore and doesn't have any SI problems.
I remember we used the rugby hold to allow him to do this (cross cradle also
works), because in the rugby hold you can also talk to them better while
maintaining a good latch. My experience anyway.

Actually re-reading your mail, sounds like this is probably not this guys
problem, but it might be worth trying to distract his hands while nursing?
It certainly won't do anyharm. If you do try it, then you need objects that
he normally wouldn't get hold of and not his normal toys - otherwise it's no
fun for him!
