Where I live, it's common practice for midwives to take on "Apprentices" (the
term I prefer over assistants), who learn by doing in addition to course
study.  Their responsibilities in- crease as they approach credentialing
expertise.  Therapist wannabees are called "interns" and are doing therapy
under supervision for 3 - 5 yrs prior to licensing, during and after
completion of coursework.  There are rules as to who is qualified to
supervise.

There are several models to choose from that may circumvent some of the
problems/issues already raised in this discussion.
