Hi Pat,

You might visit a few dermatologists in person and talk with them.  Explain
that you work with lactating mothers and sometimes need to refer them to
specialists, and that you prefer to refer to someone who is supportive of
breastfeeding and will work to maintain breastfeeding during whatever
treatment the mother requires.

See what they have to say in response and go from there.  If they are not
especially knowledgeable themselves, do they recognize that they don't know
it all and are they willing to collaborate with you?  Are they open to
updating their knowledge wrt breastfeeding?
