I caught the tail end of a webinar today from ILCA, "Supporting Breastfeeding During Flu Season: Working with the CDC Guidelines in Your Hospital" and someone mentioned near the end that we needed to be addressing the education of how to properly use the bottle and pacifier for women who choose to pump exclusively. 
I know, personally, that I've seen many working and pumping mom's get quickly overwhelmed by the amount of milk their caregivers are indicating that the babies are demanding. When the caregiver is instructed on how to pace bottle feeding and how to use the "magic baby hold" and just get a little more creative with ways to soothe the baby, then it seems to help. I also counsel moms that if the choose to use the pacifier on occasion, to think of it as a tool, but to beware of it's possible effects on their supply if overused -- especially if there are existing suppy or weight gain issues.
So, much as I personally dislike bottles and try to teach cup feeding to all my clients, I realize I must learn about these techniques to better serve them. I'm sure with all our collective experiences, some guidelines could be created.
