Can't sleep thinking about the mom, dad and baby I saw last night.  Baby is
12 days old with c-sectioned mom.  Both mom and dad are obese and mom has huge
breasts.  Baby was 8 lb 3 oz b.w., 7-9 d/c, 7-15 on day 10.  Baby was given a
bottle of formula in hospital w/o parents permission when instead of being
gone for "15 minutes" for hearing check, he was gone for "four hours"!!  When the
baby was finally returned to them, he vomited and it seemed like "formula to
them."  He also became resistant to latching onto breast shortly after that.
Mom was given nipple shield on day two because she had"flat nipples" which
don't look flat at all, to me.  Baby being fed breastmilk by bottle and has had
no other formula.   Parents went to Target and bought a pump on sale for $129
which is of course, mostly useless.  They rented a Lactina yesterday before I
got there and liked the difference.   They are low income and have only enough
money for the one week rental they paid the rental place.  Baby screams, turns
red, barely takes a breath, the minute he is put near the breast.  I would
drip some b.m. onto nipple which he would go for but then scream.  It is
completely unnerving for mom, dad and it was for me as well.  We got the baby to
finger feed, but not all that happily, and of course it is much slower for parents
than the bottles.  Up until yesterday, they were using a Playtex nurser but
nurse at ped convinced them to switch to larger Avent nipple on bottles.  Mom
really wants to breastfeed.  We tried lying down which was a worse disaster
than sitting up since baby then became inconsolable, and breast size became
unmanageable.  Mom and dad have very limited intelligence and dad insisted that
cartoons be on t.v. the whole time I was working with mom and baby in living
room!  Tried various positions with baby sitting straight up, football, cross
cradle, etc. but it made no difference as, as soon as the baby went near breast he
would mouth slightly a couple times, begin thrashing and then scream.  Skin
to skin is okay for baby as long as it is not with breast.   I want to see them
again later today as I am really worried, but am not sure what other
suggestions to come up with.  Any thoughts welcome.  Thanks, Ann Conlon-Smith, IBCLC,
