I am at home today, so I do not have the numbers with me for milk banks, but the medela breast pump kits have two booklets in the package - one is regarding breastfeeding info, the other is an instruction booklet for use of pumping equipment.  Somewhere in the middle of the equipment booklet are several numbers for milk banks.  I believe the bank in Colorado will even pay for shipping of the milk if there are 100 ounces or more.
