Subject: Looking for LC insurance billing invoice
I have been researching all my files and lactnet and can't find what I
am looking for. Do any of you have a "specific form" that you give to
your clients so they can submit to their insurance companies for
lactation consultant reimbursement? I am looking for one that has all
the lactation codings. I received several copies of a "sample" years and
years ago but can't find any of them now. I'll blame it on my move. Now
that I am doing private practice I am having clients tell me that their
insurance will pay but I need to give invoice and I wanted to provide
something professional. Do any of you ladies have such a sample letter
you wouldn't mind sharing or direct me to where I can get a copy? I
think Linda Smith has a sample form in her book about starting private
practice which is on my lists of things to purchase but I need this form
now. Please email me off list. Thank you so much!!
