Hi All,

 

Got a call from a local NP last week and just talked to mom today.  I have
permission to post her very curious situation.

 

Mom had baby #4 on 5-10.  Is having the same issue she had with baby #3 -
but did NOT have with babies 1 and 2.  

 

After baby #3 was born (I am unsure of when - there is a slight language
barrier), she experienced pain in the left breast at the upper edge of the
areolar tissue.   She was unable to even touch the area without pain.  Baby
was unable to nurse on that side without causing her pain.  However, there
was NO pain at all in the nipple - no trauma.  Mom was given a pump (I think
she said it was a Lactina) by WIC and pumped for 8 months on that side
alone.  Baby was able to breastfeed fine on the other breast.  Her Dr and
LC's thought it was a yeast issue - gave her nystatin and that only made the
issue worse.  Dr cultured, couldn't find anything, but gave her oral
antibiotics - apparently she was on them for quite some time.  

 

Baby #4 was born at 34 wks.  She immediately started pumping - and had the
same pain, in the same area as with baby #3.  There was NO pain or issue
after she weaned baby #3 nor during her pregnancy.  The area is slightly
red, she cannot touch it.  Pumping hurts as does breastfeeding on that side.
However, as with last time, the nipple is fine.  No trauma - no pain.  It is
ONLY that area on the upper part of the areolar tissue.  Baby is fully fed
and doing well on the unaffected breast.

 

I spoke with her NP last Friday and suggested another culture of the skin
that is affected, as well as rule out Paget's.  I have not seen the mother
yet.  Culture will be done Tuesday (tomorrow), and depending on results I
will see her after that. 

 

I am looking for other ideas on what this could be.  Thoughts appreciated!
