It could be nerve pain.  Some babies fall off a bit as the breast
softens, and as their latch becomes more shallow, they compress the
nipple and irritate the nerves.  Vasospasm can be the result of this
nerve and vascular compression, or pain can.  Sometimes the pain is
referred.

Have mom try and take the baby off and relatch once the pain starts.
