Does anyone on the list have suggestions/recommdentations for 
breastfeeding questions that could be asked during follow-up phone calls 
after discharge home from the hospital.  I don't want to re-invent the 
wheel, so any examples are appreciated. 

Theses phone calls would take place approximately 2 weeks after 
discharge.  We have a high breastfeeding inititaion rate 85-90% and we 
want to determine our rates 2weeks post discharge.
