Thanks to everyone who responded.  My friend went to see Dr. Smillie last week.

It was the opinion of her office that the thrush was gone (and yes, was thrush)- the white patch left 
on the tongue was milk tongue.  Dr. Smillie also thought that given the condition of my friend's 
nipple- that staph was also present at some time.  She felt that the pain this mom was still 
experiencing was from vaso-spasm related to the past infection and over-supply.  They were very 
impressed with the Breastfeeding support that she had received so far, especially with the over-
supply issues.  It was Dr. Smillie's opinion that my friend still had over-supply (even though this 
mom thought it was much improved) and that she should continue the over-supply regiment for a 
while longer.  They also helped her with some gum clenching and shallow latch issues (mostly due to 
the baby trying to control the heavy let-down).  This mother raved about how wonderful everyone in 
the office was and how well everything was coordinated.

Many thanks to all of you with your quick replies.  Please keep this family in your thoughts, 
breastfeeding is going better but the baby now has RSV.  They are caring for him at home and mom 
is extra glad that she is still breastfeeding.
