Regarding nipple pullers commercial and homemade. I would suggest you try
them out yourself before recommending them to a mom. The commercial version
has a wide base which actually puts pressure on the areola rather than the
nipple. Anecdotally, it has caused extreme pain and discomfort for some
women. It is a pinching feeling and has been known to turn the areola and
nipple a dark purple even with less pressure than is required to evert the
nipple. Youch. According to the manufacturer's re, the wide base can be
removed and flipped around so that a syringe size opening is available to
apply to the nipple. This works better for some mothers, but if you put
your finger up inside the puller, you will feel a sharp, uncomfortable
ridge. (I have really tried to make these work)

As for the home-made version or inverted syringe, They are very easy to cut
if you run hot water through them before cutting. There are instructions
available for their use. Although some institutions won't allow their use
due to liability fears, many others use them routinely with good results.
