I just recently asked this same question and was directed to the CDC
website.  The specific URL is:

http://www.cdc.gov/nchs/about/major/nhanes/growthcharts/charts.htm  It's
very helpful because you can download the individual charts or entire sets,
depending upon your needs.

There is also a lot of info in the archives about growth charts:-)
