Hi Angie,
I have a few thoughts (2 mo old originally 32 wks gest, bottlefeeding ebm, spitting up).
1. I think the mother should be supported in resuming breastmilk feeding since the doctor's idea to try other formulas did not change the symptoms.
2. Has she been able to or does she want to help baby learn to direct bf? That might help with the control of intake etc.
3. Has the baby started taking larger quantities, longer inter-feeding intervals, faster nipple, faster feeding in general, or been fed by other people, or propped bottle, or not burping as often? Or baby put down after feeds? Any of these sort of "the way he is being fed is different" things could be causing the spitting.
4. I assume baby has been evaluated in ER and at doctor for such things as pyloric stenosis, volvulus, malrotation, reflux, or other such GI disorders.
