If menstrual cycles resume with regularity within the first 6 months,
that makes me wonder what kind of breastfeeding management is being
practiced.  Most moms who are feeding on cue do not experience the
return of menses at all before six months, while this mom apparently
had at least two or three, possibly more, cycles in the first six
months -- enough that she was correlating them with repeated bouts of
mastitis.

I would go over the importance of cue feeding, exclusive
breastfeeding (did first baby receive supplements?), watching the
baby not the clock, continued night nursing (did first baby sleep
thru the night? if so was 'training' used to achieve this?), and the
normalcy of very frequent feeding in the early weeks & months.

There is also a possibility that she has a tendency toward oversupply
as well as very robust monthly hormone cycles, leading to early
resumption of menses.
In that case, the usual suggestions for oversupply would be the place to start.
