Re: My client whose milk is suddenly going away - some more info.  Mom
reports there is NO WAY she can be pregnant.  I asked "so you and your
husband haven't.,gave the 'nod' and raised eyebrows - this is a very private
woman," and she said "NO!!!  I won't let him!!"  So, not pregnant.  Also no
birth control pills of any kind.  We did discuss the possibility of the
psychological issue and she said that may be part of it - but is still
worried.  She has a right to be.  Also no big 'nasty' happening around that
time.  No major stress that she felt comfy to report to me other than the
timeline of losing her milk last time and the family history of the same.

 

I told her to get to the Doc and let's get some blood-work done.  Someone
mentioned a list of tests to look at - thank you!  I'll ask for those.  :-)
And, hey - if y'all haven't figured out by now I have no issue violating
that SoP when I need to!  There is no history of any scheduling of feeds and
I can't do a suck eval to rule out baby issues because he thinks my finger
is a really cool play-toy!  He is 4 months old.

 

Bigger issue than that tho is baby refuses to take a bottle or a cup.  There
is observational evidence of feeding issues in the way of control.  Mom
works very hard (too hard?) to get baby on the breast.  He arches and
protests when put into feed position.  Several times I said gently, let's
just let him sit and be happy while we talk some more.  This baby was
clearly hungry, but flat out refused to eat until he was ready - which was
when he was dead tired.  Then he nursed quickly (2.4 oz) and crashed out
completely.  However, he woke up about 20 minutes later but again, refused
to nurse even tho he appeared to be showing hunger cues.  Mom has been so
worried about his feeding that she has been force feeding him with a dropper
out of a sense of panic.  The family basically pins him down so that she can
get the dropper in his mouth and make him eat 2 oz of milk this way.  I was
horrified!  I didn't show it tho!   No wonder he is arching and resisting
feeding.  He is not interested in a bottle or a sippy cup or anything that
he would need to do something with to get milk - I tried all my tricks today
with him.with no luck at all.  He was happy to play with the bottle nipple
in his mouth but flat out refused to suck on it.  Kid is a challenge but
hella cute!

 

We used the SNS and he would latch with that, but again only when he was
dead tired and then he didn't get a lot from that because he just petered
out.  So she will keep working with that.  She will also be using the scale
to monitor his intake as it is nowhere near where it needs to be.  We'll get
her pumping to see if that will bring up the milk supply as well.  I just
don't know for sure if this is a baby issue (weak suck due to very fast
flow) or a mom issue (milk supply drying up).  Also unclear is moms supply -
it seems to be high at one feeding and then not the next - not consistent at
all.  One feed baby gets 2oz the next nothing.  Something just isn't right
here.  Mom reports that everything was great up until last week.

 

We talked about keeping feeds as HAPPY as possible!  Lots of skin to skin.
This baby is not getting enough milk and I am concerned and unsure how to
write up my Dr report and describe the control issues along with the
possibility that we may have a serious feeding issue developing with this
baby.  Any words of wisdom would be very helpful here.  This is one I
haven't had to deal with - the issue of feeding issues developing under
these circumstances.  

 

 

Ok - lastly, regarding the Silent 3000 - THANK YOU!!!  Thank you for
speaking up - Thank you for your comments.  One gal mentioned how the SoP
really does affect everyone regardless of whether or not they are an IBCLC.
Thank you for that as well because, frankly, I agree with you!  So - Thank
you!  You are amazing!

 

Ok - I am done now.oh wait - my mommy group was FABULOUS!  I had so much
fun!  I can't wait to do the next one!  And what was really cool is that 4
of the mommies really wanted to know about the Infant Massage classes I
teach.  I told them I was setting up a schedule for Jan - they looked at me
like I was nuts!  So I asked if they wanted to start at the next mommy group
(right after it actually) and it was a resounding YES!  SO I am dancing on
Cloud 12 right now 'cause Cloud 9 ain't high enough!  :-)
