The ENT could clip the tongue-tie, and help with his breathing and nursing.

If the tongue is held in the bottom of the mouth, it is also held in the
back of the mouth, made worse by the micrognathia ( recessed chin)  Good
idea getting him sitting up.  You might even try laying him prone on mom to
nurse, gravity will help the tongue drop forward out of the airway.

I had a kid like this a few years ago...the frenulum was very thick, and I
was not willing to clip it.  But a local ENT did.
