Breastfeeding Abstracts, Aug 1995 has an article
"Feed the Babies" by Susan C. Nicholson MD and Mark S. Schreiner MD.
They are both anesthesiologists.
The basic conclusion of the article is that clear fluids are acceptable
until 2 hours before anesthesia.  Breastmilk was treated the same way in
some studies.  There are references included.
