Doesn't requiring a Rx give it status? Like an antibiotic, it is life saving liquid?  I think a printed sheet on how much to give and how to gently wean from the supplement and why there is a need to wean from formula would be more appropriate.  One of the writers amongst us could come up with a share-able form..... It isn't needed just post discharge, it is needed in peds office for  whenever they say give a supplement. 
 
Heard a terrible tale the other day, very PT  baby given only breast milk, exclusively during hospital stay.  Home one day, weighed by ped, who noted no weight gain since hosp (24 hours), different scale!  Told mom to add 1 tsp of formula powder to bottles of mom's own milk, which she did, baby now has NEC :-(  Pat in SNJ
