This sounds very suspiciously like the presentation of Paget's disease.
The nipple retraction is concerning.  Must get a biopsy. "Let's watch it"
or "try multiple creams" is NOT an option. Her dr is correct that it needs
to   be checked out ASAP.  Remember that the biggest problem with breast
cancer in pregnancy or lactation is the delay until  diagnosis!  No one
wants to think that a young pregnant woman may have breast cancer, but
