       Several thoughts come to mind, so here they are in no particular
order.  Some mothers just don't do well with a particular brand of pump.  Any
chance she could switch to some other brand?
       Are the breastshields large enough?  If her nipples scrape back and
forth along the sides of the nipple shaft, then the suction is not able to
release and apply appropriately, so the milk may not come out well.  She may need a
larger breastshield.
       Have you taught her hand expression?  Some mothers just don't do well
with electric pumps.  Some do better with a good manual pump, like Avent (no
financial interest).  Others do best with no pump, just hand expression, once
shown how to do it properly.
       Perhaps the firmness in her breast is due to engorgement rather than
lots of milk.  Several times moms have been able to pump more if they wear
cabbage leaves for 15 minutes *before* they pump.  This reduces edema, which seems
to take the pressure off the ductwork, so the milk can actually get through
the ducts.  Usually, the moms I've worked with have only needed to do this
once, and then the milk moves better on a continuing basis.
       You mentioned using warm towels.  Have you placed these on breasts?
That might actually increase edema and pressure on ductwork.  If you haven't
already done so, you could try placing the warm towels on her shoulders while
she pumps, or place them behind her so that when she sits back in her chair the
warm towel lines up behind her breasts.
