Several years back, at an ILCA Conference in Boca Raton, we got to observe the ultrasound studies showing which really changed the perception of the internal breast structure.  While they were saying "we don't see any (structures that could be called) lactiferous sinuses, the ultrosound was placed over the area not far behind the nipple-the same place earlier drawings/studies identified the sinuses.  This was showing the ducts filling and expanding, but no noticable structure when they were not filling or full.My thought then and now was/is, Ok, so they are not firm like a bottle that fills and empties, but more like baloons that expand and contract.  It is still in the same general area that the "filling" ocurs.  So I came up with the (definitely tongue in cheek) term PWYPYF for that location meaning Place Where You Put Your Fingers (when expressing milk).  Of course, most expression techniques actually place the fingers behind this location, but
 the point is, whether they are identifiable structures or not, the location is what is importnt as it seems to be the region where milk collects prior to removal whether by baby or by hand or by machine.
So the point is, it isn't critical whether we believe in the existence of actual milk sinuses or not.  It is understanding where and how things are happening in the breast that affects how we act.
