Robin,
You might suggest that mom latch baby #1 first, and support her with pillows,
then use both hands to get baby #2 on.  #1's suckling will help provide an MER
for #2, and mom can give her more support.  Perhaps mom would be willing to
let baby #2 sleep with her at night to facilitate more nursing, "until she
fattens up".
