Most of the mothers I have seen with very large nipples also had fairly flat, firm breast tissue. Even if the baby could put the nipple into his mouth the breast wasn't pliable enough to compress the sinuses. The most recent patient was so large that only the hospital grade pump could create enough suction for expression. There is no way a nipple shield would even fit.

I think we need to be cautious about recommending herbal remedies because of the unmonitored manufacturing and non-standardization of doses. My allergist strongly recommends against eccinachea for those of us with pollen allergies, etc. You never know about cross-over reactions.
