The hypothyroidism is a big red flag here.  Mom needs to make sure that
her thyroxine level is well within the normal range on medication, and
she needs to stimulate her milk supply with pumping and perhaps
medication.  Prolactin levels are probably not going to be helpful, but
stimualting prolactin will be.

And if it hurts and the baby is not getting enough milk, all bets are
that the latch is poor.  Yeast is possible, but least likely to be
responsible for her pain with this history.  Shallow latch and low milk
supply work together to cause nipple damage.  She needs that IBCLC as
soon as possible.
