I do not have a policy, since I work privately, but I do know that the
problems always come when nipple shields are handed out as a replacement
for skilled breastfeeding help, without adequate followup by a skilled
lactation professional.

This is what originally caused the huge backlash against nipple shields a
few years back,  namely inappropriate use. Nipple shields are a tool, like
a screw driver or hammer. They are appropriate if you need that tool, but
not if you need another, and of course, the skill is in choosing the tool
and the method. The real issue I find is that many people are unaware of
good latch technique, and this is the real problem that causes untold pain
and suffering of mothers everywhere. Since I answer about 350 breastfeeding
questions per month, I can tell you that I speak from experience.
