This mom sounds like she probably has polycystic ovary syndrome, all the
way up through the pregnancy complication of pre-eclampsia-- has she
ever been formally assessed and diagnosed?

Definitely a difficult situation. The anemia needs to be treated, of
course; that is a known factor with insufficient milk production and
important for her health in general. Was she put on iron supplements? At
the least she will need guidance on high-iron foods that are
bioavaiable, and don't forget that vitamin C/foods when combined with
the iron sources helps absorption.

In addition to this, she might benefit from metformin (used to treat
PCOS), domperidone, and/or goat's rue. I would advise extreme caution in
using Maxolon (metocloramide) with this mom because that drug can induce
depression and she is probably at higher risk because of her hormonal
problems. I've seen the depression develop as quickly as "the second
round" twice the past year (MDs prescribed, not my recommendation). Any
of the above treatments would probably need to be long term because she
most likely has a primary vs secondary supply problem.
