Laurie:  I have always used a phone log.  Ours is a ledger and is made for 
me by the same company that does our birth and nursery logs.  The company 
is:  Physicians Record Company  3000 South Ridgeland Avenue  Berwyn, IL  
60402-0724   USA  Telephone number:  800-323-9268.
Each page is divided into columns labeled:  Date, Time, Caller, Phone 
Number, Baby's Age, Concerns, Interventions, Follow-up.  This method has 
served me well for 20+ years.  When the log (which is hard bound) is full, 
it can be stored.  I am only half-way thru the Log I started 4 years ago, 
so they last a long time.  There is space for 25 phone calls/page.
You tell the company what you need, they lay it out for you, and send you a 
proof for your approval, and print up the final draft.  Very nice and 
convenient.
