It seems to me that this issue has been discusssed previously but I have
been unsuccessful in my archival search, please bear with me....
I have seen a mom with 3 week old c/o bilateral nipple soreness, born in
another facility but was not helped much with breastfeeding there.  The
baby is a product of a fairly short labor, but pushing lasted two hours
with no progress (possibly due to presentation) and fetal distress which
was resolved with caesarean section - infant reportedly had significant
bruising to frontal area of head. Weight loss exceeded 10% and
pediatrician opted to supplement with formula using syringe method
toward the end of the first week of life; after which the infant gained
like a champ, and has regained to birth weight just now. The mother
reports that feedings are painful due to nipple tenderness, every
feeding.  She has begun to pump (has a full milk supply) and bottlefeed
ebm to her babe on occasion to rest her painful tissue.
The problem: This baby has a smallish mouth with large tongue, which
protrudes slightly at rest.  When she feeds, she literally scoops the
breast into her mouth latching incorrectly, she also clenches her jaw
rythmically as part of her suck cycle.
We corrected some basic positioning technique with no improvement.
Suggestions?
This mom is past ready to quit.
