Then there's also the issue of PCOS in obese women. The current thought
is that PCOS is triggered by a defect in carbohydrate metabolism that
causes the insulin resistence and obesity, and results in long menstrual
cycles, decreased fertility, and sometimes low milk supplies. Since
Lisa Marasco has publicized this issue, I've seen many moms with PCOS
and very low milk supplies. I imagine an adopting mom with PCOS will
also have a high risk of lowered supply.

For normal weight moms inducing lactation for adopted babies, I have had
one have a complete milk supply, one have 75%, and several have about
half. The remainder made drops, and a few made none at all. The moms
who made the most milk were those who had had repeated first trimester
pregnancy loss, the moms with none at all had never been pregnant and
had primary infertility issues. These results were from before
domperidone became available in our area.
