Judy brought up my question re: the IBCLC providing "preventive counseling." She wrote, "What I do, and what many
IBCLCs do is more like "crisis intervention" and feels a lot like trying to stop a train that has gone in completely the wrong direction or derailed completely.... very time-consuming and ultra-detailed consulting."

Yes! That's what I do. Will these consults be billed differently? Many moms I see may have already seen a couple IBCLCs already. This last little family I'm working with I saw twice in one day even. Is there going to be a different code for this different type of client? (what's the opposite of "preventative?") I suppose we are preventing death, but I don't think that's the way the term is to be used. :)

Another question..will the mom be limited to one visit per day, meaning can she see an IBCLC on the same day she sees her physician? And if we see the mom *with* another clinician, can both bill for services?

I know this is a passionate debate, particularly looking at earning an viable income. I will throw out there that living in Idaho, more than 70% of the population where I live is below poverty. I absolutely don't consider myself in the category of a "you get what you pay for" IBCLC. I'm thankful to not be the bread winner; yet I'm pretty much "stuck" in a part of the country where income will never match (for example) the rates in NY--even though my skills warrant such. I think this is a good opportunity to learn about reimbursement, approach it slowly (each from our individual p.o.v.), and to remember it's not a mandatory opportunity.

Warmly,
Amy Peterson, IBCLC
