I was puzzled along with you, until you cleverly mentioned thrush.  Of
course!  The baby was probably "eating strangely" because his mouth felt
strange.  Maybe not painful, maybe just a strange taste, but enough to make
him change the way he ate.  It's no surprise it didn't show up on the
culture; thrush doesn't always show.  I'm really impressed that you came up
with the idea.

I saw one little baby who was maybe 6 weeks post-cardiac surgery.  He had
pretty clearly demonstrated that he'd rather starve (literally) than do
anything besides just nurse at a bare breast, despite what doctors had told
her to do, and things started going well for them once she listened to him.
But then he started refusing to eat again.  Within a few days, her nipples
were sore.  Sure enough, when they were treated for thrush he started eating
again.  No symptoms except her pain and his breast refusal.  Not so very
different from your own case.
