I'm not certain about the "required" part of your question, but suspect that the answer can be as varied as the type of coverage each individual policy is providing.  It is my own experience in my private practice that some do and others do not.  Even more confusing, some may choose to provide it as a cost-effective, group "education" interaction and others may require it as provate, "individual" counseling - which basically translates into a prenatal consultation.
