I think the substance is likely the normal secretions of the Montgomery
glands. Due to the piercing, there is likely a fistula and, I am just
speculating, that the normal secretions collect there rather than get
distributed naturally onto the surface. If she is very concerned and this
could affect how she feels about bf, a culture could certainly be done and
a pathology smear to see what the cells are.
