Thanks to all who responded. I really appreciate the support.

An update:
The mother went to see her ped this morning (I had faxed him my report
before she arrived). On his scale the baby weighed 7lbs 4 oz with a diaper
on (why not weight her naked????) So the ped told mom that he is encouraged
that the baby is gaining weight well.

Mother states that the baby has been waking more and drinking better from a
bottle. In fact, she actually woke once on her own and cried in the middle
of the night. Mother and father were ecstatic about that. She still is
only taking approx 1 oz at a time, though. Mom has been keeping track of
intake and for all of Sunday (a 24h period), baby took approx 14oz formula.
Hard to believe she gained over 4oz from Sat p.m. to Mon a.m. on that!
Mother states that baby is still somewhat lethargic and has to be woken for
most feedings.
So... watch and wait, I guess. The ped wants her to bring the baby back
each day this week for weight checks, so hopefully if there is anything
further going on it will be caught. Per mom, he believes it is just a case
of a baby who got too weak to eat and as she gets more into her, she will
'perk up'.

Now, regarding mom's milk supply which is pretty much nil at this point...
She has been using a Lactina (which is all I carry and nothing else was open
on the weekend).. the flanges are too small (30mm, mind you) and pumping is
painful. In fact, she states that her nipples are hurting all the time and
baby isn't even latching. I'm taking a Whittlestone over later this
afternoon and we'll try that to see if there is any improvement. I'll be
taking my scale as well :-)
Mom picked up some herbal supplement at the local health food store which
she says contain fenugreek and blessed thistle, among others. I'll look at
it when I see her this afternoon.
If the Whittlestone doesn't do anything for her, I'll look into getting the
glass flanges from Medela and having her use a Classic.
Anything else anyone can think of?

We'll work on latch when baby is up on weight and alert and mom has some
milk. Right now, I feel that with mom's nipples so large and baby so small
comparatively, it would be like batting our heads against a wall.

Thanks again,

Cindi Swisher, RN, IBCLC
