I work alongside an SLP.  It has taken years to build communication and trust--well worth the effort.  Here are some practical mental choices that helped us:  Have clearly defined scopes; understand each others' objectives; have "permission" to question each other and brainstorm options before deciding a care plan (in front of the mother respectfully); ask each other "would you be comfortable trying..." and then why or why not; have a timeline of when things are expected to improve and when the care plan might shift; be comfortable with what you don't know; keep an open mind and trust the other department to "do their job" and allow you to do yours as well.
