Dear Experienced ones, I would like to hear your evaluation of this scenario. Info in mom's own words (with a few additions) in response to my questions. Permission to post:

"41 year old mother.  Third child (son) born in May 2009 at 37 weeks with antigen C incompatibility.  Infant under bilirubin lights for 4 days, separated from mother immediately after birth.  Mother pumped breast milk and fed in bottle, along with formula prescribed by NICU. Mother attempted to breastfeed when home with infant, but only had success the first few weeks at nightime.  Milk supply was low from the first.

{Previous children - girl, born 1996 (hard to latch, good milk supply) died of SIDS 1997; girl, born 1998 (latched at 7 weeks, good milk supply) breast fed for 4 years.}

Lost milk supply at three months, regained supply with Domperidone (40mg/d), fenugreek (12 caps/d) (pumping) but only to 4-6 ounces a day.  Baby has never latched since early months.

On no (other) medications, including birth control, cycle normal.  Pumped with an electric double pump in the hospital, but used a hand pump once home.  Milk did not come in as with previous children.  Electric double pump was resumed at three months.  No diabetes, no heavy bleeding, but was anemic during pregnancy, now taking Fe supplements.  Difficulty maintaining pregnancy for ten years before becoming pregnant with this baby.  Would conceive and then miscarry at 6 weeks, three or four times a year for 10 years.  This baby, as with previous two, was a c-section.  This baby was in the breech position and was a difficult birth."

"Diagnosed with PCOS in 2005. Took metformin in 2005-2006. metformin was given because even though (she) did not have cysts, the doctor felt that [there were] had enough other symptoms to suspect PCOS.  Had consulted dr because of weight gain and miscarriages, was tested for thyroid (function) during pregnancy and also immediately after, all normal.

Mom wants to know: Is her age more of a factor in low supply or is the PCOS more of a concern.  And, if there is a subsequent pregnancy, what are the recommendations of the lactation consultants?"

How could this mom increase her supply over the 4-6 ounces per day?
