Poor baby got use to the immediate flow from the bottle, so I usually try a
SNS to coax them back to breastfeeding.  The mom will set it up so the flow
is immediate, then turn it off when she sees him gulping, or feels a
letdown.  Also offer breast every 1 1/2 to 2 hours all day so he comes to
the breast-a-rant before he gets really starving and desperate.  If he is
desperate, then offer the bottle for 1/2 oz or so, then work with him at
the breast.  It should always be a positive experience for him.
Even if he has just had a bottle, let him sleep on/near the bare breast, so
he learns to love that closeness, and it will increase mom's milk due to
the higher oxytocin level when baby is skin to skin!
