I have a young client with Turners, and one of the distinctive physical
features is that the nipples are widely spaced on the chest.  I have no idea
how this slight anomaly plays out during later breast development, but
knowing that, I would tend to be concerned about function.  I would at least
watch closely during lactogenesis, and might anticipate delays in onset of
copious milk production by use of a pump if baby seemed disinterested in
latching and sucking.  I have seen babies whose mothers merely had flat or
inverting nipples  enter a real quick spiral into disinterest in sucking
which contributed to delayed lactogenesis.  And while most babies can
generally wait til the milk comes in, some can't (ie if small to begin with,
or if other problems exist) and none can wait forever for the milk to come
in.  So while we don't want to intervene too quickly in normal situations,
it is still our responsibility to be aware of risk factors for delayed
lactogenesis, and to be prepared to step in to help the mother  protect the
baby and the milk supply if needed.  It's still the same old song of
individual assessment and case-by-case management, and in spotting the diff.
between normal and not.
